Bridges to Health

Beginning July 1, Vermont’s Free & Referral Clinics became the fiscal sponsor of Bridges to Health. Bridges to Health is a state-wide Community Health Worker program that addresses barriers faced by workers and their families, preventing them from reaching their full health and well-being potential. Bridges to Health is committed to developing, coordinating, and implementing community-based outreach initiatives and services which contribute to health and health equity within communities at local and statewide levels. Our programs include grant-funded and philanthropically-supported projects.

This gift will provide invaluable support for sustaining health access services for hard-to-reach and medically underserved populations through care coordination, food security, insurance enrollment, and community-based health programming across Vermont.

Please note that the legal name of Vermont’s Free & Referral Clinics is the Vermont Coalition of Clinics for the Uninsured. This is the name that appears on our donation platform and acknowledgments. Vermont’s Free & Referral Clinics is our registered assumed name.
